For some reason, the first commit fails:

Hmm. I experienced the same messages as you, but a second commit seemed to work fine (see below). I'm using the same version of Junos OS. I requested a reboot and logged on to the system using the newly changed password.

I think it's hdd virtualization problem. Possibly, too slow write operation. I'm using qemu over vmware for juniper emulation.

I think you may be right. I created a Juniper VirtualBox VM and the recovery commit occurred without any errors.

The odd part is that Juniper on QEMU works fine and commits changes reliably when the system is running normally. In other words, the error seems to only occur in QEMU when the Olive is booted in Recovery mode.
